I was only mildly surprised to find Holmes’s latest client waiting for us in a carriage outside my door. Holmes has always expected me to drop everything at a moment’s notice and come along on one of his cases. Why should he think the obligations of marriage and a growing practice would get in the way? They didn’t after all.

I'd say that Holmes was always a little unsure if Watson would really agree so willingly to any of his undertakings, but Watson simply never put that into question, so... no need for uneasiness. =P

They certainly did not. I like to think Watson realized, at least subconsciously, that following Holmes was following his calling. He was born to do this, to be a detective's loyal partner and a writer of their adventures.
