Very speculative, and you're welcome to disprove it...But I wonder if the original nurse died, and Mrs Munro couldn't find an adequate substitute? Mr Munro calls the nurse he meets 'coarse'; but Mrs Munro's description of her old nurse doesn't sound coarse. Scots, after all.
