During the train ride home, I sat quietly, contemplating my companion. He’d proclaimed our country holiday a success and looking at his smiling, happy face, I couldn’t disagree. Yet when my eyes chanced to linger on the livid marks just visible above his collar, I shivered. Was the day coming when I might not be in time to prevent disaster?

As a doctor, Watson probably thinks a lot about remedies, and the risks they impose in return for the benefits they give. This is great to see here, and the place so well chosen - a train compartment, in which Watson would have the perfect space and time to study his friend. Well, and agonising himself, which he could perfectly turn off by strapping Holmes to his chair or something. ;P

Oh so tragic, foreseeing a dark future we already know so well. That's one of the interesting things about reading the cases in alphabetical order -- we drift through time knowing the future in many ways better than the past, as readers. The characters,though, are chained to time in an inexorable line.

If I should suggest a title for a possible new field of study/publication to the dear doctor, it'd be "The Science of Distraction". I am sure it would be the medical breakthrough of the decade, if not century... please excuse me, I have studies to tend to. *lol*

Nice foreshadowing, as well as expression of what must have been Watson's thoughts on the journey home. Holmes shrugs the attack off in the story, but it was obviously severe and undoubtedly took some time to recover from.
