I read the situation as they wobbled upstairs.

The thick, fresh bandage on Watson's patient's hand spoke of recent severe injury. He'd been sent to the local physician in that condition as he was barely walking on his own. Probably a train case. His story impressed Watson enough to immediately bring him to me himself.

An attempted murder, then.
